Description

POSITION: Youth Community Support Worker
DEPARTMENT: Community Counseling (KC Northland)
Salary: $ 42,000- 48,000 $ (Annually)

RESPONSIBILITIES:

  • Provide community based services to assigned caseload, utilizing independent judgment and
  • decision making regarding appropriate treatment needed.
  • * Provide Crisis intervention to consumer's experiencing a psychiatric or substance related crisis
  • and assess and determine the intervention needed including the need for hospitalization or residential treatment.
  • Maintain ongoing contact with hospitalized clients including participation in staffing and discharge planning so that consumers can successfully reintegrate into their living environment.
  • Complete progress notes in AVATAR within 72 hours of service provided that clearly reflect interventions provided and necessity of those interventions.
  • Complete the Initial CPR Assessment on all consumers within 30 days of entering into services and the annual assessment thereafter.
  • Responsible for maintaining daily billing expectations.
  • Coordination of care with treatment providers and other agencies.
  • Develop Person-Centered treatment plans initially and annually, which are strengths based, reflecting the consumer's and family's life goals and individualized choices according to DMH/agency timelines. Work in collaboration with Health Care Home team to provide assistance to clients regarding their overall physical health by using wellness coaching techniques.
  • Complete initial and annual Crisis Intervention Plans, Rights and Responsibilities, and all other required paperwork in a timely manner.
  • Maintain knowledge of agency technology programs and/or systems needed to do the job and uses them effectively.
  • Maintains required training and certifications.
  • Maintains required agency productivity standards.
  • Participate in team meetings and quarterly all staff meetings.
  • Other duties and/or responsibilities may be assigned by supervisor

Qualifications:

  • Education Level: Master's degree
  • Other minimum requirements: Must be able to provide direct service to clients.
  • Years of Experience: 1-3 years post related experience.
  • Must be at least 21 years of age and pass background check, physical and drug screening. This position also requires a valid driver's license and proof of current vehicle insurance.
